More than 1 in 3 adults in the US have prediabetes. Of those with prediabetes, more than 84% don’t even know they have it.

What is the National Diabetes Prevention Program?

Developed by the Centers for Disease Control and Prevention (CDC), this lifestyle change program aims to prevent or delay the onset of type 2 diabetes among individuals with prediabetes. The program provides evidence-based curriculum and support to help participants make sustainable changes in their diet, physical activity, and overall lifestyle, with the goal of achieving and maintaining a healthy weight and reducing the risk of developing diabetes.

Are You Prediabetic?

You may be at risk for type 2 diabetes or have diabetes if you:

  • Are 45 years of age or older
  • Are overweight
  • Have a family with history of type 2 diabetes
  • Are physically active fewer than 3 times per week
  • Ever had diabetes while pregnant (gestational diabetes) or gave birth to a baby that weighed more than 9 pounds
  • Without weight loss or moderate physical activity that helps lower blood sugar levels, many people with prediabetes can develop type 2 diabetes within 5 years. Type 2 diabetes is a serious health problem that can lead to health issues such as heart attack, stroke, blindness, kidney failure, or amputations. Now is the time to take charge of your health and make lasting changes to prevent or delay type 2 diabetes.


What Do I Get By Joining The Program?
  • A full year of support!
  • Sessions with a trained Lifestyle Coach to help you eat healthier, be more physically active, manage stress, and solve problems that can get in the way of healthy change.
  • Realo offers 16 weekly to bi-weekly sessions during the first 6 months of the program, then monthly sessions the last 6 months of the program.
  • All programs are held virtually by Zoom or by phone.
  • Participants may be eligible to receive a free water bottle, digital scale, and fitness tracker for participation.
  • This program is valued at $430, but through grant funding, Realo is able to provide the Diabetes Prevention Program absolutely free!
